Led team that executed Blue KC's Patient Centered Medical Home (PCMH) program. The Medical Home program during 2012 - 2014 was a shared savings model that evolved into an attainment model. Provider entities were provided a monthly care coordination PMPM payment and were monitored monthly on quality and efficiency measures as well as overall total cost of care for their attributed lives.
