There are many situations in which funny things are happening when you switch the languages being used.

Most of the time I'm having problems with saying in what language I received an information, for example if the article I read was in English or Polish.

Another funny thing is confusing languages. I just got a situation that I started posting here and when getting back I realised it's written in Polish. I didn't realise it while writing. I guess it was caused because a second before I was checking Polish translations on a dictionary.

The same happens when I talk or hear two languages. When three are involved it's totally out of controll :D I had a situation where a German colleague was asking his colleague about something in German, which I generally understand, then he was translating it to English sometime mixing English words with German ones and I was trying to say it in one language, preferably Polish, to a Polish hotel receptionist on the other side of my phone.

I also knew a girl who was unconsciously switching from English, which she knew rather poorly, to German, which was quite good, while actually speaking a sentence. Try to imagine faces of her conversational partners, not knowing German of course, when they were seeing that she actually expects an answer.

Yes, when interpreting in public I repeat the sentence in the same language as it was said (translation back and forth;)

I sometimes speak English to my mom after talking to my husband :) or I mix some Polish and English words while talking to my husband but that's more like code-switching.

For me, quite similar.

Additionally, I have an issue where I get tired, or anxious, then I am unable to create sentences in Polish. Not even simple ones.

I thought we never forget our mother tongue, even if we are exposed to a second/third language; studies show that people instinctively count, swear, and pray in their native languages even when living in another country (might not be true for everyone). After years of communicating in Spanish and English, I still find it easier to write and speak Polish.
What really, really gets me is when, during a job interview, the candidate starts speaking Polish with an accent (!) or says, "Lubię, kiedy moi uczniowie są tacy... eager." Being bilingual is all bout the ability to use both languages effectively without making an ass out of yourself. Workplace jargon is a different thing though.

Funnily enough, and I think I must have mentioned it some time ago, I get more and more British vocabulary and grammar in my English (because I suck up to Warren, obviously). Rafal W probably remembers the "trainers" I got him for Sean :D

For me, there is almost no problem with confusing languages. But ONLY when I throw my previous language's dictionary away of my head. And I need about 10-15 minutes of speaking one language to forget previous one :)

I'll remember that next time when I can't help saying something in English in the middle of a Polish conversation only because the English phrase sounds 'rounder' - that's a terrible habit and there are no excuses for it. I never do it in writing - on the contrary, when there is a Polish equivalent of an 'international' word, I always take care to use it in translation. But in spoken conversation I'm sometimes quite guilty of it. Before long, I'll be like that builder I overheard while in London, shouting: 'ej, podaj mi ten wheelbarrow!' ;)

One of my friends used to ask me to accompany him to the home of a patient (Polish guy checking the feet of a geriatric diabetic patient), an old black guy.

My friend couldn't process the Urban Black English. Incidentally, that guy was a former Harlem Globetrotter. It was a pleasure to grab his newspaper and listen to him admonish me to "make yourself at home, boy..."

i have a problem switching from English to German if I am using both in a specified time.
for instance, talking in German to one and then English to the other - I usually start speaking German English to my German friend + I have forgotten most of my German, which pisses me off as hell and you know what? I obviously have no time to improve this shit
- then again you might say it's so Polish to complain an' do nuthin' but that's the awful truth.

I really wish a day could have 48 hours...
